Why We Built This
Finding adult day care for a parent or spouse is harder than it should be. Most families don't know where to start — a Google search returns a mix of paid lead-generation sites, incomplete state agency lists, and facilities that may or may not still be operating. Calling around is time-consuming, and the stakes are high: you're making a decision that will affect your loved one's daily life.
Adult Day Care Locator exists to make that search faster and more trustworthy. We built a single, comprehensive, publicly accessible directory of every adult day care provider registered in the federal NPI database — over 11,000 facilities across all 50 states — organized by state and city, with addresses and phone numbers so you can contact centers directly.
There's no sign-up, no lead form, no fee. You find a center, you call them. That's it.
Who We Help
Our directory is used by several groups of people, each with slightly different needs:
- Family caregivers — often the primary audience. Adult children caring for aging parents, or spouses managing care for a partner with dementia or a chronic condition. They need to find options quickly, compare what's available in their area, and contact centers without going through an intermediary.
- Social workers and case managers — professionals who help clients identify community care resources. Having a complete, verified list of providers by geography saves significant research time.
- Hospital discharge planners — when a patient is discharged and needs a daytime care solution while they recover, discharge planners use the directory to identify local options quickly.
- Older adults themselves — many people research their own care options. The directory is designed to be navigable by anyone, including people who prefer to make their own decisions about care.
- Facility owners and administrators — many use the directory to verify their own listing, identify competitors, and understand the local market.
Our Data: Source and Methodology
Where the Data Comes From
Every listing in our directory is sourced from the CMS NPPES NPI Registry — the National Plan and Provider Enumeration System maintained by the Centers for Medicare & Medicaid Services. This is the official federal database that assigns National Provider Identifiers (NPIs) to all healthcare providers and organizations operating in the United States.
We filter the full NPPES dataset for providers with taxonomy code 261QA0600X — the Healthcare Provider Taxonomy Code specifically designated for Adult Day Care programs. This taxonomy is self-reported by the provider when they register with CMS.
What "NPI-Verified" Means
When we say a listing is "NPI-verified," we mean the provider has an active National Provider Identifier in the federal NPPES database and has self-identified as an Adult Day Care program. NPI registration is required for any healthcare organization that conducts electronic health care transactions, including those billing Medicare or Medicaid.
NPI verification confirms that a provider is (or was, at the time of registration) a federally recognized healthcare entity operating as an adult day care program. It does not replace state licensing verification, and we always recommend families independently confirm a center's current state license before enrolling a loved one.
Data Accuracy and Updates
The NPPES registry is a live federal database that providers are required to update when their information changes. We periodically refresh our data against the current NPPES export. However, real-world details — hours of operation, whether a center is currently accepting new participants, specific services offered — change more frequently than federal registrations.
We encourage all families to call ahead to confirm details before visiting. If you find a listing that has incorrect contact information, has permanently closed, or has moved to a new address, please email us and we'll update or remove the listing promptly.
How to Use This Directory
- Search by state or city. From the homepage, enter your city and state or select your state from the map. You can also browse directly to any state page (e.g., /ny/) and then drill down to a city.
- Review the listings. Each city page shows all NPI-verified adult day care centers in that location, with their address, phone number, and care model (social, medical, or combined) where available.
- Call directly. There's no inquiry form standing between you and the provider. Click the phone number or call directly — the center's staff can tell you about availability, services, pricing, and Medicaid acceptance.

Yes — completely free for families, caregivers, and professionals. Searching the directory, viewing facility details, and contacting centers directly costs nothing and requires no account or sign-up. Our goal is to make this information as accessible as possible to everyone who needs it.
For Facility Owners
If your facility is listed and has incorrect information — wrong phone number, outdated address, or the wrong care model — we want to fix it. Email us at hello@adultdaycarelocator.com with your facility name and NPI number and we'll update the listing.
If your facility is not yet listed, it may not be registered in the NPPES database under the Adult Day Care taxonomy code. NPI registration is free and available at nppes.cms.hhs.gov. Once your NPI is active, contact us and we'll add your listing to the directory.
